Course overview

The module will be delivered by academics and practitioners in the School of Health Sciences, who have a wealth of expertise in long term conditions and care coordinating caseloads.  Clinical specialists will also have input into the teaching of this module.

Module outline and aims

This module aims to provide nurses with an introduction to the care and management of clients with long term conditions, with a focus on primary care and community settings.

Content outline

Indicative content includes:

  • Asthma/COPD/Spirometry
  • Diabetes Mellitus
  • Mental Health and Mental Capacity
  • Adult Safeguarding
  • Cardiovascular disease
  • The use of emerging technologies
  • Understanding the therapeutic alliance and active support for self-management
  • Case management and care coordination

Prerequisite knowledge

Registered Nurses current on the NMC register

English requirements
