Providing supportive and palliative care at the end of life is a challenge in many health and social care settings. The role of the hospital is significant due to the number of deaths occurring there annually. However, a further significance of inpatient care is that up to a third of inpatients admitted at any given time are likely to die within one calendar year of their admission (Clark et al 2014). Mortality rises sharply with age, gender and medical speciality (as compared with surgery). This module focuses on patients in the last year/s of life including those living with advanced disease in hospital settings, who have end of life or palliative care needs and who may be in hospice, community health or social care settings.
