Did journalists fail the residents of Grenfell Tower?

A debate around reporting of the tragedy in which at least 70 people died. Why weren't fire safety concerns widely reported before the disaster and what lessons can be learned from coverage during and afterwards? What are the wider concerns about the decline of local newspaper journalism in London?
