Course Content

This module aims to provide community nurses with an introduction to the care and management of clients with long term conditions. It recognises the importance of joint training of health and social care staff as set out in The Future of Primary Care – Creating Teams for Tomorrow (Health Education England 2015) and provides an opportunity for shared learning between practice nurses and community nurses.

The module will be delivered by academics and practitioners in the School of Health Sciences, who have a wealth of expertise in long term conditions and care coordinating caseloads.  Clinical specialists will also have input into the teaching of this module.

Indicative content includes:

  • Asthma/COPD/Spirometry
  • Diabetes Mellitus
  • Mental Health and Mental Capacity
  • Adult Safeguarding
  • CHD/Hypertension
  • Perspectives on telecare/telehealth
  • Understanding the therapeutic alliance and active support for self-management
  • Case management and care coordination

10th, 24th May; 7th, 21st June; 5th July 2018

Eligibility

Registered Nurses current on the NMC register

English Requirements

Applicants must be proficient in written and spoken English.

For those students whose first language is not English one of the following qualifications is also required:

  • IELTS: 7.0

Please note that due to changes in the UKVI's list of SELTs we are no longer able to accept TOEFL as evidence of English language for students who require a CAS as of April 2014.
