What will I learn?
The short course will enable you to:- Identify recent publications that have evaluated the best ways to reduce hospital re-admission
- Understand the ways in which one local team have reduced admission through hospital at home services
- Discuss the different ways in which your local team can reduce hospital re-admission rates.
